We consider it a special privilege to provide quality care for your children as they grow and mature from infancy to adulthood, and we’re dedicated to helping you through all the stages in between.
We’re proud to be your Primary Care Home and are here to coordinate your child’s care to help you get the services you need when you need them. We’ll listen to your concerns and answer your questions, help you play an active role in your health and offer after-hours help and alternatives to the Emergency Room.
As a Primary Care Home we’ll make prevention and wellness a top priority. If you have a special health concern or condition, we have a team that’ll help connect you with other professionals and resources. This team includes Care Coordinators, a Referral Specialist and Behavioral Health Specialist. Your health care team is led by your primary care provider and we’ll all work together to ensure you get the best possible care.
